    Anambra accident: FRSC boss condoles with victims’ families

    The Corps Marshal, Federal Road Safety Corps, Mr  Dauda Ali Biu, has condoled with the families of victims of the crash along the Onitsha -Inteje-Akwa axis in Anambra on Friday.

    This is contained in a statement by the Corps Public Education Officer (CPEO), Mr Bisi Kazeem, on Friday in Abuja.

    Biu, however, called on road users,especially fleet operators, to exercise caution and be conscious of their environment whenever they are driving.

    He urged motorists to avoid driving against stipulated speed limit on all roads, saying it was necessary for all to take caution.

    Biu said adhering to safety measures were important to help the corps eradicate or minimise incidences of a road accident caused by impatience and lack of mental alertness of motorists.

    “This advice came following the fatal crash that occurred involving a Mitsubishi L300 commercial Bus with vehicle registration number XE 245 AWK and a DAF Leyland Truck with registration number T-19094 LA at the aforementioned route.

    “According to the first information report from the scene of the crash, eyewitnesses stated that the DAF Leyland truck was coming from Onitsha while the Mitsubishi L300 bus was coming from Nteje axis.

    “8  vehicles got to the failed portion at the same time when the container of the Leyland Truck fell on the Mitsubishi Bus, ” he said.

    The accident, which involved 10 people, killed five female adults, injured two male adults and two female adults.

    The Corps Marshal commended FRSC operatives for their prompt response to the distressing situation.

    This, he said, ensured immediate evacuation of the victims to  the hospital for medical attention and clearance of obstruction, allowing free flow of traffic within the axis.

    Death toll in Niger’s boat accident rises to 30

    The Niger Emergency Management Agency (NSEMA) on Monday said the death toll from Sunday’s boat mishap in Mokwa Local Government Area had risen to 30 from 24.

    Alhaji Garba Salihu, NSEMA’s acting Director  General, made this known in an interview in Minna.

    He said that the agency had so far rescued 30 persons in Gbajibo area where the incident occurred.

    ”Yesterday, I told you that we have recovered 24 corpses, but that same day it later increased to 26 dead bodies.

    ”Today, we recovered four more corpses, making 30 in all,” Salihu said.

    The acting director general said that 27 of the deceased were women, while three were male children.

    Salihu dispelled the rumour making the rounds that 100 passengers boarded the ill-fated boat.

    “For now, nobody knows the number of passengers on that boat. Many people entered the boat, so knowing the actual number of passengers on board will be difficult,” he said.

    Salihu said that the agency had stopped searching for the victims of the boat mishap, pending when the need arose.

    The victims were from Gbajibo, Ekwa and Yankyade communities who were on their way to their farms at the other side of river Niger (old Gbajibo) when the incident occurred.

    The Federal Road Safety Corps (FRSC), Ondo State Command, said two people died, while five others were injured in an accident on Friday night at Olokuta area on the Akure-Ondo road.

    Mr Ezekiel SonAllah, the Sector Commander of FRSC in the state, confirmed the incident to NAN in a telephone interview on Saturday.

    SonAllah, who explained that the accident occurred at about 5:48p.m, said it involved six male adults and one female adult.

    The sector commander attributed the cause of the crash, which involved one commercial vehicle and a Honda Accord car, to speeding.

    “On Friday at about 5:48p.m at the Olokuta Correctional Centre area along Akure-Ondo road, a Nissan Primera commercial vehicle marked FGB-96XA and a Honda Accord car marked MUS-834 AL were involved in an accident.

    “Two male adults died on the spot, while four male adults and one female adult were injured.

    “The deceased have been deposited at the Ondo State Specialist Hospital morgue in Akure, while the vehicles have been handed over to the police,” he said.

    SonAllah admonished motorists to always abide by the traffic rules and regulations, saying any violation would be arrested and prosecuted.

    The police has recorded three deaths in a multiple accident at Dutse, Baupma in Abuja.

    “The FCT Police Command is deeply saddened to report a tragic multiple motor accident that occurred on the 8th of July, 2023, at approximately 12:30 PM in Dutse Baupma. Upon receiving a distress call from a concerned citizen near the Tipper Garage, our dedicated traffic personnel promptly responded and rushed the victims to Kubwa General Hospital for urgent medical attention.

    “Regrettably, we confirm the loss of three precious lives, as confirmed by the attending doctor. The deceased have been identified as follows: Hosea Stehen, a 34-year-old male from EFAB Metropolis Estate, Karasana, Abuja; Muhammad Lawal, a 32-year-old male from Jere, Kaduna State; and Bashir Shaibu, a 25-year-old male, also from Jere, Kaduna State.

    “Our thoughts and condolences go out to the grieving families and loved ones during this incredibly difficult time.

    “Additionally, four individuals involved in the accident are currently receiving medical treatment at Kubwa General Hospital. They are Maria Ibeku, a 21-year-old female; Ebuka Eleodimuo, a 38-year-old male; Joy Adebayo, a 36-year-old female; and Bashir Haruna, a 25-year-old male. These individuals hail from Mbouko Village, Dutse, and Karasana Village in Abuja.

    “Furthermore, four vehicles involved in the accident have been safely secured at the police station. They include a Black Toyota Camry with registration number YAB 355 AH, a Toyota Pilot (registration number unknown), a Truck (registration number unknown), and a Tricycle with registration number KUJ 571 VE. It is worth noting that a Toyota Jeep with registration number BWR 468 CD, believed to belong to a Department of State Security officer, was taken from the scene by DSS Personnel from Ushafa, Abuja. The driver of the vehicle has been identified as David C.Y Audu, a male resident of Ushafa.

    “The FCT Police Command urges the public to remain calm and patient as we conduct a thorough investigation into this unfortunate incident.

    “We assure you that every effort is being made to ascertain the causes and circumstances surrounding the accident. The Commissioner of Police extends his deepest sympathies to the families, relatives, and loved ones of the deceased.

    “We will continue to provide timely updates on any further developments related to this case.

    3 dead , 2 injured in Abeokuta-Sagamu road accident

    Three persons were confirmed dead and two others injured on Saturday in an accident involving a Rio car and a Sino truck on the Abeokuta-Sagamu expressway in Ogun.

    Mr Babatunde Akinbiyi, the Spokesperson of Ogun Traffic Compliance and Enforcement Corps (TRACE) confirmed the incident to newsmen in Abeokuta.

    Akinbiyi explained that the accident occured at 9:44 a.m. and was caused by excessive speed and loss of control on the part of the Rio car driver.

    He said that the accident occurred near Day Waterman College and involved a black Rio car marked AKD 827 DV and a white Sino truck with registration number WDL 466 XA.

    He said that the car with four church officials was coming from Oke-Aragbiji in Osun.

    According to him, eyewitnesses said the driver lost control due to excessive speed, ran over the median, hit and uprooted the street light before colliding head-on with the truck loaded with cement.

    “The truck was coming outbound Abeokuta.

    “Instantly, the three men of God died due to the impact of the collision while the driver and the passenger in front were injured,” he said.

    Akinbiyi added that the injured victims were evacuated to Ijaye General Hospital in Abeokuta for treatment and the deceased deposited at the hospital morgue.

    He stated that the two vehicles had been taken to Owode-Egba Divisional Police Command for investigation.

    The TRACE spokesperson commiserated with the deceased families, calling on motorists to eschew excessive speed and always take adequate rest before embarking on a journey.

    Bureau releases preliminary report on Max Air aircraft incident

    The Nigerian Safety Investigation Bureau (NSIB), on Thursday in Abuja, released preliminary report on the serious incident involving a Boeing 737-400 aircraft operated by Max Air Limited with Nationality and registration marks 5N-MBD

    NSIB Director-General, Mr Akin Olateru stated that the incident occurred between take off at Yola, Airport Adamawa, and on landing at the Nnamdi Azikiwe International Airport, Abuja Nigeria on May 7.

    He said the initial findings show that the flight crew are licenced and qualified to conduct the flight and the aircraft has a valid Certificate of Airworthiness.

    “At 12:10 h 5N-MBD departed Nnamdi Azikiwe International Airport (DNAA) for Yola (DNYO) as NGL1648 and arrived Yola at 13:25 h.

    “The aircraft was on ground Yola for about 30 minutes before turning around for Abuja. According to flight crew, after takeoff, landing gear was left extended for 3 minutes to allow cooling due high temperature on ground Yola.

    “At 14:08 h, the Nigerian Air Force personnel from the Air Force Hanger close to runway 35, reported to Yola Control Tower that an object appearing like a tyre fell off from the departing NGL1649.

    “Yola DATCO promptly informed the Pilot of NGL1649 as well as Kano Area Control (ACC) and Abuja Tower, “ he said.

    Olateru said the flight purser (L1) and another cabin crew (L2) informed the Pilot that a passenger at seat row 24 informed them about seeing a wheel fall off from the airplane.

    The NISB boss said that the pilot mentioned instruments and landing gear indications were normal.

    He said Yola ATC informed Abuja Tower and NGL1649 that nothing was found after the runway inspection at 12.51h.

    “According to the pilot, the landing was normal but after the airplane started decelerating he noticed the port side wing was dropping lower than normal.

    “He applied full aileron control to keep the wing from dropping until the lift decayed. Thrust reverser and minimal braking was used to stop the aircraft on the runway.

    “At 14:57 h, Abuja ATC informed NGL1649 pilot that should be advised it appeared he had smoke around his aircraft that fire watch room had been called. Aerodrome Rescue and Fire Fighting Services (ARFF) and emergency services were alerted. “

    The director-general said the aircraft came to a stop between Link A5 and A6 about 2900 m from the threshold of runway 22.

    According to him, the port wheel assembly was located by personnel of the Federal Airports Authority of Nigeria (FAAN) about 1000 m from threshold of Runway 35 Yola airport on May 8.

    He stated that the roller bearings on the wheel assembly of No. 1 main wheel had discoloration and was burnt out.

    “The CVR recording was found to be overwritten. Max Air Limited stated that they were unaware of the NCAA All Operators Letter (AOL) (NCAA/FSG/AOL/19/03) in respect of continuous overwriting of Cockpit Voice Recorder (CVR) Information.

    “There was wrong tech log entry of serial number of incident aircraft. The quality control department did not supervise maintenance action of the brake unit and wheel change on 5N-MBD conducted on May 5.

    “The crew did not take adequate measures to confirm the information about the missing wheel assembly, “ he said.

    The director-general affirmed that there was severe fire damage on the No. 2 main wheel assembly.

    According to him, after the replacement of the wheel brake on May 5, an aircraft mechanic crosschecked the work performed by the engineer.

    He said the Yola ATC Voice Recording System (VRS) did not record any communication between the pilot and the duty controller on the day of the occurrence.

    He said that the Max Air Limited Quality Assurance ought to ensure maintenance activities were supervised and monitored in accordance with Maintenance Control Manual for immediate safety recommendation.

    According to him, Nigerian Civil Aviation Authority (NCAA) should conduct regular Safety Management System (SMS) assessments to evaluate the capability of Max Air limited to effectively manage safety.

    “ Ensure that PMI and POI of Max Air Limited increase surveillance and oversight to ensure maintenance and flight operations are carried out in accordance with NCAA Technical Guidance Material (TGM) Volume IV (Airworthiness Inspector’s Handbook) Chapter 23 (Continuing Surveillance).

    “Also, TGM Volume III Flight Operations Inspector’s Handbook Chapter 30 (Continuing Surveillance).

    “Max Air Limited should ensure that flight crew display good situational awareness and airmanship at all times during conduct of flight operations in accordance with Max Air ops manual., “ he said.

    According to him, NCAA should invoke relevant sections of Nig.CARs 2015 in accordance with IS 1.3.3 Legal Enforcement Actions to non-compliant operators with the AOL (NCAA/FSG/AOL/19/03) on continuous overwriting of Cockpit Voice Recorder (CVR) Information.

    He advised the NCAA to ensure FAAN and Nigerian Airspace Management Agency (NAMA) provided uninterrupted power supply in order to avoid vulnerability of their operational equipment.

    “Further investigations inspection of the roller bearing and other accessories , retrieval of pertinent document from Max Air limited and other stakeholder and pertinent factors leading to drafting of the final report, “ he said.

    Tragedy as 103 marriage guests die in Kwara

    Police Command in Kwara has confirmed that 103 persons died in a boat accident at Egbu village, in the Patigi Local Government Area of Kwara.

    The Spokesperson of the Command, SP Ajayi Okasanmi, confirmed the incident on Tuesday.

    The deceased reportedly met their end after the boat conveying them while returning from a marriage ceremony capsized.

    The tragic incident has cast an ominous cloud of sorrow and despair on the village.

    Okasanmi said the names of the survivors would be published soon.

    The Transition Implementation Committee Chairman, Patigi, Alhaji Mohammed Liman, put the number of passengers that died in the accident at 110.

    Liman, while quoting from a statement by one Alhassan Bala Mohammed, a community leader in the area, said, “About 110 people died in the boat mishap in our area.

    “We’re going to tell you about the tragedy and unforgettable memory that occurred to my people from our village and other neighbouring villages close to us.

    “Our village people went to a wedding ceremony in another village called Gboti and the boat carried about 270 people, and almost 110 people were lost.”

    “This is an unforgettable memory and tragedy in the history of our village,” he said.

    Kwara Speaker sympathises with Patigi people over boat mishap

    The Speaker of Kwara House of Assembly, Mr Yakubu Danladi-Salihu, has sympathized with the Etsu Patigi, Alhaji Ibrahim Umar Bologi II, and the people of Patigi over the Monday’s boat mishap.

    In a statement by the Speaker on Wednesday, he condoled  the families of the dead and identified with those still missing, praying to God to forgive the deceased and give the families fortitude to bear the loss.

    The boat conveying people from Egboti in Niger State en route to Kpada in Patigi Local Government Area of Kwara, capsised on Monday night, leaving several people dead and others still missing.

    “Our heart goes out to the people of Patigi, especially those in Ebu, Dzakan, Kpada, Kuchalu, and Sampi over the boat mishap that claimed dozens of lives and left others missing.

    ”We also condole the Etsu Patigi over the unfortunate development; we pray to Allah to grant the deceased eternal rest and give their families fortitude to bear the loss,” he said.

    The speaker also tasked the Fire Service and the rescue team to do everything possible to ensure the recovery of those still missing alive.

    Actor Treat Williams dies after motorcycle accident

    Treat Williams, the actor best known for his role on “Everwood,” died in a motorcycle accident at age 71.

    His death was confirmed on Tuesday by his agent Barry McPherson.

    Around 5 pm (2200 GMT) Monday, Williams was riding his motorcycle on a Vermont road when a car cut him off.

    “He was killed this afternoon,” McPherson, his agent of 15 years, said.

    He was making a left or a right and a car cut him off.

    “I’m just devastated. He was the nicest guy. He was so talented.”

    Williams was lifted to a hospital by helicopter sent from Ticonderoga in New York, but could not be saved.

    From 2002 to 2006, Williams led the WB series “Everwood” as the kindly Dr. Andy Brown.

    More recently, he was seen as Lenny Ross in “Blue Bloods.”

    He gained acclaim at age 28 in the movie version of “Hair,” earning a Golden Globe nomination for Best New Star.

    He went on to “Prince of the City,” “Smooth Talk” opposite Laura Dern and “Things to Do in Denver When You’re Dead.”

    In all, Williams racked up more than 120 film and television credits over a four-decade career that was still going strong.

    He is survived by his wife, Pam Van Sant, and their two children, Gil and Elinor Williams.

    Intending pilgrims of Nasarawa state in North central Nigeria were involved in a road traffic accident while on transit to Nnamdi Azikiwe international airport Abuja enroute to the kingdom of Saudi Arabia for this year’s Hajj exercise.

    Sources from the state pilgrims board said the accident occured today (Wednesday) at “Kara” (Animals market) in Keffi local government area of the state saying they were moving to Abuja ahead of their departure scheduled for Thursday 25th May, 2023.

    However, sources privy to this development said no life was lost during the accident and the injured persons are receiving medical attention at a Health facility in the state.

    … as FRSC blames speed violation

    No fewer than 14 people were killed in an auto crash that occurred on Thursday at Zangoro Village, along Bauchi-Darazo Road in the Darazo Local Government Area of Bauchi State.

    Five others sustained various degrees of injuries in the crash that involved two vehicles.

    The state Sector Commander, Federal Road Safety Corps, Yusuf Abdullahi, confirmed the incident to our correspondent.

    Abdullahi said the crash occurred around 12.17pm.

    He said, “The crash occurred at Zangoro Village along Bauchi-Darazo Federal Highway and it involved one commercial vehicle and a private one.

    “They are a yellow Golf 3 Wagon with number plate AJ-507 GWA and an ash Chevrolet with number plate 15B-07BA belonging to the Bauchi State Government.

    “A total of 19 people comprising 10 female adults, seven male adults, one female child and one male child were involved.

    “Immediately our men arrived at the scene, they rushed the victims to the Abubakar Tafawa Balewa University Teaching Hospital, Bauchi, for confirmation and medical treatment. Unfortunately, 14 of them were certified dead by a medical doctor upon arrival there.

    “The deceased included two male adults, 10 female adults, one male child and one female child. Those who were injured are five male adults.”

    While identifying speed limit violation as a possible cause of the accident, Abdullahi said N73,000; seven mobile phones, one power bank and four small handbags were recovered at the scene of the crash.

    He said the corpses had been deposited at the morgue of the ATBUTH, Bauchi, after which they would be released to the relatives for burial.

    He added that the five others who sustained various injuries are receiving treatment in the hospital.
