    How to stop crashes on Ojuelegba bridge – FRSC boss

    The Corps Marshal, Mr. Dauda Biu has expressed concern over recurring container crashes on Ojuelegba bridge in Lagos and called for barricades to stop articulated vehicles from using the bridge.

    Biu said this in a statement issued by the Federal Road Safety Corps Public Education Officer, Mr Bisi Kazeem on Sunday in Abuja.

    He was reacting to the fatal crashes that occurred at Ojuelegba Bridge in Lagos in which nine people died and another that claimed 11 lives at Soka bridge on Lagos-Benin Highway.

    He said that the construction of the barricades would provide permanent solution to the recurring crashes.

    Biu also called for the expansion of connecting roads under the Ojuelegba bridge to accommodate articulated vehicles.

    He revealed that FRSC rescue teams in collaboration with other emergency agencies had cleared obstructions from the two crash scenes for ease of movement.

    Biu added that the driver of the truck that caused the Ojuelegba accident was arrested and handed over to the police, while efforts were on to arrest the driver that caused the Soka bridge crash.

    The corps marshal reiterated the need for enforcement of laws that would bring justice to victims of traffic crashes.

    “Efforts should be made to activate them so that justice will be served accordingly to both the drivers and the victims,” he added.

    The corps marshal cautioned commuters against route violation and driving against prescribed legal speed limits.

    He urged the public to call the FRSC toll free line 122 and National Traffic Radio 107.1 FM to report any traffic emergency.

    SAD! Two Indian fighter jets crash, pilot killed

    Two Indian Air Force fighter jets crashed on Saturday, killing one pilot, in an apparent mid-air collision while on exercises south of the capital New Delhi.

    The crash involved a Russian-made Sukhoi Su-30, carrying two pilots, and a French-built Mirage 2000, operated by a third.

    Both aircraft had taken off in the morning from the Gwalior air base, around 50 kilometres (30 miles) east of where they came down.

    “The aircraft were on routine operational flying training mission,” the country’s air force said in a statement, adding that one of the three pilots had sustained fatal injuries.

    An investigation was underway to determine the cause of the crash, it added.

    Police officer, Dharmender Gaur, told AFP from the scene of the crash that another pilot had been found alive but injured in the forests of Padargarh around 300 kilometres (185 miles).

    “We have located the wreckage of one of the planes,” the officer said. “The other plane has likely fallen further away from the site and we have sent teams to locate it.”

    Local authorities had been instructed to assist with the air force’s rescue and relief work, Madhya Pradesh Chief Minister Shivraj Singh Chouhan tweeted.

    “I pray to God that the pilots of the planes are safe,” he added.

    The crash is the latest in a string of aviation accidents involving India’s military air fleet.

    Five army soldiers were killed last October when their helicopter crashed in Arunachal Pradesh state, near the country’s militarised and disputed border with China.

    It was the second military chopper crash in the state that month, coming weeks after a Cheetah helicopter came down near the town of Tawang, killing its pilot.

    India’s defence chief, General Bipin Rawat, was among 13 people killed when his Russian-made Mi-17 helicopter crashed while transporting him to an air force base in December 2021.

    10 die in road crash on Lagos-Ibadan expressway

    No fewer than 10 persons were confirmed dead on Saturday in an accident involving a Mack truck and a Toyota Hiace bus at Oniworo area on the Lagos-Ibadan expressway.

    Mr.  Ahmed Umar, the Sector Commander, Federal Road Safety Corps (FRSC) in Ogun, confirmed the incident to newsmen in Abeokuta.

    Umar explained that the accident occurred at 4: 30 a.m.

    The FRSC boss noted that the accident was caused by road obstruction on the part of the trailer and also excessive speed and loss of control on the part of the driver of the bus.

    He explained that the Toyota bus marked KLD 539 XA lost control and ran into the truck with no registration number at the rear.

    ”A total of 19 people were involved in the crash which comprised 13 male adults and six female adults.

    ” Two male adults sustained injuries while seven male adults, two female adults, and one female child died in the accident,” he said.

    Umar stated that the injured victims were taken to Victory Hospital, Ogere for medical attention while the deceased were deposited at Ipara morgue.

    The sector commander urged motorists to always put on the headlamp when the weather is poor.

    He cautioned motorists, especially truck drivers, to always display caution signs whenever there was breakdown on the road.

    He commiserated with the families of the crash victims and asked them to contact FRSC Ogunmakin for more information about the crash.

    Nigerien military helicopter crashes, three killed

    Three people have been confirmed dead after a Nigerien military helicopter crashed on return from a routine training flight.

    The country’s defence ministry said the helicopter crashed while landing at an airport in the capital Niamey on Monday, December 26, killing three people on board including an expatriate instructor.

    The wreckage burst into flames, blocking rescuers from reaching the crew.

    Reuters reported that a commission of inquiry has been set up to determine the cause of the accident.

    Tanzanian plane crash-lands into Lake Victoria

    A Tanzanian plane has crashed into Lake Victoria in Tanzania while trying to land at the nearby Bukoba airport.

    Tanzania Broadcasting Corporation (TBC) says about 15 passengers have been rescued in the crash. The degree of the crash remains unknown.

    Rescue workers and local fishermen are on the scene searching for survivors.

    The aircraft, Precision Air, was flying from Dar es Salaam to Bukoba via Mwanza when it reportedly encountered a storm and heavy rains before it crashed into the lake which has its shore near the airport.

    According to the BBC, photos show the plane almost completely submerged with only the brown and green tail fin above the water, surrounded by rescue workers and fishing boats.

    Tanzanian President Samia Suluhu Hassan called for calm during the rescue operation.

    “I have received with regret the information of the plane crash of Precision Corporation in Lake Victoria, Kagera region. I send my condolences to all those affected by this accident. Let’s continue to be calm while the rescue operation continues while we ask Allah to help us,” she tweeted in Swahili.

    A helicopter carrying pilgrims to Kedarnath in India’s Himalayan region crashed on Tuesday, killing all six on board, a police official said.

    Two pilots and four pilgrims died in the crash in the northern state of Uttarakhand, its police chief, Ashok Kumar, told Reuters by telephone, adding that a rescue team had been sent to the site.

    Footage from Reuters partner ANI showed smoke billowing from a spot nestled in between some hills, surrounded by dark clouds.

    It is too early to determine the cause of the accident, but poor weather could be a factor, Kumar added.

    Kedarnath is a key pilgrimage site that is shut every winter.(

    22 confirmed dead as two military choppers crashed in Congo

     

    Spokesperson for the Congo Army has confirmed the death of 22 people after two Ugandan military helicopters crashed in East DRC, on Wednesday.

    Uganda had sent troops to its central African neighbour in December to help fight a violent rebel group known as the Allied Democratic Forces (ADF).

    “The Uganda People’s Defence Force (UPDF) have not yet told us the circumstances of the crash,” the spokesman said without providing further detail.

    UPDF spokesman, Felix Kulayigye confirmed two helicopter crash and said “there were fatalities, but I don’t have any extra details at the moment”.

    The ADF is among several militias wrangling over land and resources in Congo’s mineral-rich east over the past decade, a conflict that has claimed thousands of lives and displaced millions.

    Military confirms top General, 5 others killed in helicopter crash

    Pakistan said Tuesday rescue teams found the wreckage of a military helicopter that crashed overnight due to bad weather in flood-hit southwestern Baluchistan province, killing all six senior officers on board.

    The crash occurred when the officers were on a mission to assess the damage that the flooding had caused during the several weeks of torrential rains.

    The heavy rain had caused a lot of damage in the province of Balochistan, a military statement said.

    The preliminary investigations suggested bad weather had caused the crash, the statement said, quelling speculation that the helicopter might have been targeted by rebels in one of the volatile regions.

    A general commanding the military in the country’s southern region and another one heading the coastguard forces were among the dead.

    The Federal Road Safety Commission (FRSC) on Tuesday confirmed the death of two persons following the explosion of a petrol tanker at Umunya Junction on the Onitsha-Awka expressway in Anambra.

    FRSC’s Sector Commander in Anambra, Mr Adeoye Irelewuyi, confirmed the incident in Awka.

    Irelewuyi said attributed the accident, which occurred at 10.00a.m. to speeding.

    “The fatal crash involved an unidentified driver of a Mack tanker with no registration number, which was laden with Premium Motor Spirit (PMS) and a shuttle bus, with no registration number.

    “According to an eyewitness account, the tanker driver was on speed, lost control, and rammed into the shuttle bus from behind. It crashed and exploded, and both vehicles caught fire.

    “The crash involved five male adults. Two were killed, including one that was burnt beyond recognition.

    “Police personnel from Oyi Division in Nteje were invited and the dead victims were handed over to them by the FRSC team from Nteje Unit Command,” he said.

    The sector commander noted that the state’s fire service had put off the fire, adding that the FRSC rescue team were still on ground managing traffic and ensuring that the obstruction caused by the crash was removed.

    While commiserating with the families of the dead, Irelewuyi warned motorists to avoid speeding and ensure they keep to the recommended speed limit.

    “Speed thrills but kills; check your speed and not your watch. Let us keep our roads safe from accidents,” he said.

    AIB releases new final accident reports, includes Opebi helicopter crash

    The Accident Investigation Bureau (AIB) on Thursday in Abuja, said it had recorded a total of 72 accidents since its establishment in 2007.

    AIB Commissioner, Mr Akin Olateru, made the disclosure when he released accident reports for the period between 2018 and 2020.

    Olateru said the report was released as part of AIB’s mandate of investigating aircraft accidents and incidents with a view to enhancing air safety.

    He, however, noted that while the Bureau had since its establishment in 2007, recorded 72 accidents, the current administration had released a total of 53 reports till date.

    “The reports of today contains a total of 12 safety recommendation, which are addressed to the regulatory body, the affected airlines, the Federal Airport Authority of Nigeria (FAAN), among others.

    “The bureau has now released 232 safety recommendations since inception with 151 of the recommendations released by the current administration, “ he said.

    He said the report was released as part of AIB’s mandate of investigating aircraft accidents and incidents with a view to enhancing air safety.

    “ The reports to be released today include the accident involving Quorum Aviation Ltd., Bell 206B3 Helicopter with registration 5N-BQW, which occurred at Opebi, Lagos State on Aug.28, 2020.

    “The accident involving Skypower Express Airways Nigeria Ltd., Air Tractor AT-401B Aircraft with 5N-BTV, which occurred at Rukubi Village 90NM South-East of Abuja on the Oct.4, 2018.

    “The serious incident involving Nigeria Police Air Wing Bell 429 helicopter with registration 5N-MDA, which occurred at Nnamdi Azikiwe International Airport (NAIA), Abuja on Aug. 3, 2019,“ he said.

    He said that there was a serious incident involving Skypower Express Airways Nigeria Ltd. Cessna 172 Aircraft with registration 5N-APE at Bini Village, Niger State on Nov.19, 2019.

    Olateru also released a report on the ground collision involving a Central Bank of Nigeria (CBN) Truck with registration AKD765FK and Nigeria Air Force (NAF) Bell 412 helicopter with registration NAF 600 which occurred at NAIA.

    He said the causal factor of the accident involving Quorum Aviation Ltd., Bell was fuel exhaustion at low altitude and low speed leading to loss of control in flight.

    He said the causal factor of the accident involving Skypower Express Airways Nigeria Ltd., Air Tractor AT-401B Aircraft was an undetermined engine failure at low altitude that necessitated execution of emergency forced-landing on a farmland.

    He attributed the causal factor of the serious incident involving Nigeria Police Airwing Bell 429 helicopter to lack of communication to update the crew on the current serviceability status of the ground handling wheel prior to arrival at DNAA.

    Olateru mentioned that the causal factor of the serious incident involving Skypower Express Airways Nigeria Ltd., Cessna 174 Aircraft was the shift of the engine oil seal at the top of the push rod.

    He said the causal factor of the bulleting report on the ground collision involving a CBN truck and NAF Bell 412 helicopter was the driver`s misjudgement of the clearance between the truck and the helicopter main rotor blades.
